TransforMED launches new physician networking site

TransforMED has launched the latest tool to aid physicians in transforming their practices into patient-centered medical homes. Delta-Exchange is a private, collaborative Web site where primary care practices can confer on projects; discuss issues, challenges and successes; ask questions; share practical knowledge and experiences; and work together to maintain the momentum for implementing TransforMED’s recommended practice enhancements.

For a $30-per-month member fee, participants have full access to this online community and its resources: online discussions, interactive webinars, how-to articles on practice improvement topics, an expert panel of practice facilitators, and customizable forms that streamline policies and procedures. Users sign up based on their “work zones,” which allow the different groups—physicians, clinical staff members, office staff members and residency programs—to have meaningful discussions about their specific needs.

“We listened to physicians across the country, and this is what they said they wanted—a solution that is cheap, doesn’t require a long-term commitment and lets practices ‘dip a toe in the water’ to get a feel for practice transformation,” TransforMED President and CEO said Terry McGeeney said in an interview with AAFP News Now.

Noting that transformation has not been easy for physician practices, TransforMED developed the site in the hope that bringing practices together and allowing them to learn from each other will ease some of the growing pains.
