DSHS offers up to $10,000 in grants for medical home initiatives

The Department of State Health Services and Children with Special Health Care Needs Services Program will give up to $10,000 in grant support to each eligible pediatric and family medicine practice to improve medical homes for CSHCN beneficiaries and their families. The CSHCN Program defines medical homes as an effective approach to identifying and coordinating the information, care, and ongoing community-based support activities required to meet patients’ medical and other developmental needs.
CSHCN beneficiaries are children or youth who have or are at risk for chronic physical, developmental, behavioral or emotional conditions and who typical require more health services than other children, according to the Notice of Open Enrollment. The organization estimates that 42 percent of these children do not receive coordinated, ongoing, comprehensive care through a medical home due to lack of physician knowledge about medical home concepts and other communication and care coordination factors.
To be eligible for the grant, physician applicants must be enrolled as a contractor for medical home supports, and sign, submit and complete an application, available through the link below. The enrollment period begins March 1, 2008 and ends March 1, 2009, during which applicants will apply for one of three six-month contract periods. Funds will be distributed on a first-come, first-served basis up to the total amount of available funding.
