Educational Objectives
After completing in this educational activity, the participant should be better able to:
- Describe the elements of the Chronic Care Model.
- Give 3 examples of how to use a diabetes registry to implement one or more elements of the Chronic Care Model.
- Explain how implementing the Chronic Care Model in their practice will improve control of risk factors for diabetes complications: glucose, blood pressure and lipids.
Target Audience and Method of Participation
This activity is designed for physicians and other health professionals with an interest in the management of diabetes, the Chronic Care Model, and participating in a diabetes registry.
This activity is a one hour video lecture with a slide presentation recorded at the 2009 C. Frank Webber Lectureship on March 5, 2009. In order to receive credit for this online activity, participants are required to view the activity and complete an evaluation survey at the end of the activity to verify participation. Upon completion of the activity and evaluation survey, participants will be able to print out a certificate of participation.
